We usually consider the pituitary gland to be the externalization of the forehead center or ajna... and the pineal gland of the crown center or sahasrara. I understand the actual "third eye" to be the major head centers when they are all active, at least to some extent, and functioning together as one big "head center." It is said that when the lights of the major head centers join together, there is a large and bright light in the head. This "head center" could be said to look out through ajna (and thus the eye pictured in the forehead) and to look upward through sahasrara. The DK/Bailey books say the third major head center is the Alta Major, and its externalization is the carotid gland. I was taught to open a person's third eye by directing energy through the forehead, through ajna/pituitary, through sahasrara/pineal, and then angling downward... which would connect with the Alta Major.
